Athletic Forms and Physical Information
All participants in Cannon Athletics must fill out 3 forms:
These are due on the first day of practice for that season. The physical must be valid through each season of participation. Physicals are typically valid for one year from the time of the physical. In order to be covered for the entire year, the best time to have a physical done is the summer.
You can have a physical performed at your family doctor or our medical athletic trainer provider, Novant Health, provides free physicals on the morning of June 2, 2018. The screening is for rising 7th -12th grade athletes.
The link to register for the 2018 Novant Health Competitive Edge Sports Screening is www.ppezoom.com. Please note that parents must pre-register their child to attend; day-of registration are not allowed. Registration closes on May 27. Choose CANNON from the choices of school.

Register for Bus Service
Cannon School is pleased to offer bus service to all students, subject to space and routing limitations.
Advanced and AP teachers will communicate directly with students in the coming weeks about summer readings and assignments in their respective courses. All summer reading books and course materials for next year will, once again, be available for purchase from our school’s online bookstore. We will contact you when the bookstore opens, around the second week of July.
