Follow the steps below to complete the required Back-to-School Forms for the upcoming school year, 2023-2024.

General Information about Magnus Health
Cannon School partners with Magnus Health to collect and securely store your student’s health and medical information. Cannon also uses Magnus to store all required athletic and medical forms. Parents may use the Magnus web portal or the Magnus Mobile App to access all forms, fill in student health information, and upload forms that need a signature.
Timeline for Submitting Health Information
We ask that you enter the required health information for each student’s Student Medical Record (SMR) no later than Monday, July 31, 2023.
Timely submission of health information and all required forms is vital to be in compliance with NC laws and to assure everyone is prepared for your child’s first day of school.
How to Access Magnus Health
Returning parents may begin to complete and upload forms to Magnus Health beginning on June 3, 2023.
New parents may begin to complete and upload form to Magnus Health beginning on July 17, 2023.
All parents: Prior to the dates above, any forms required for participation in summer athletics should be completed and emailed to Cannon’s Health Office and Cannon’s Athletic Trainer. All forms are linked within this letter, below, and may be downloaded and printed for your convenience.
In mid-July, all parents will begin to receive reminder notifications from Magnus Health alerting you that action must be taken on your student’s account. These reminder notifications list any incomplete requirements.
Please use this link to log into your Magnus account using your My Cannon Username and Password.
How to Upload Forms to Magnus Health
You may upload forms to Magnus using any of the methods below:
- Photo: Take a picture of the form and upload the JPEG directly to Magnus online.
- Scan: Scan each document to your computer as a PDF and upload to Magnus.
- Fax: Fax forms to Magnus and they will upload the forms for you.
- Use the New Magnus App (The Magnus Mobile V2 version)

REQUIRED HEALTH FORMS & INFORMATION

- Vital Health Record: Electronic Form – log onto Magnus to complete online.
The vital health record is an electronic form that must be completed and updated annually. If there are any changes to your child’s health history, if you move, change your phone number, or if an emergency contact changes throughout the year, the vital health record is where you will need to make the changes.
- North Carolina Summary of Immunizations: Printed Form, and/or physician’s Immunization Record
In accordance with the North Carolina General Statutes (G.S. 130A-152 (a)), all immunizations for Cannon School students must be current before the start of the school year. If a student's immunizations are not on file with Cannon School within thirty calendar days after the first day of school, the student will not be allowed to remain in school or participate in sports until the required immunizations have been completed.
Complete the form by filling in the dates of your student’s immunizations or attach their Immunization Record given by their provided.
Junior Kindergarten/Kindergarten students must have their 5-year-old or Kindergarten immunizations to begin school. Please see the full list on Magnus.
7th Grade: All students entering the 7th grade or who turn 12 years old before August 17, 2023 must have both the Tdap and Meningococcal conjugate vaccines.
12th Grade: All students entering the 12th grade or who turn 17 years old before August 17, 2023 must have the second dose of the Meningococcal conjugate vaccine.
● Kindergarten Assessment: Printed Form
Required assessment prior to the first day of school. Print form, have it completed by a physician, and upload Magnus.
Cannon School Medication Policy:
- All medications must be in their original packaging with the original prescription label.
- All prescriptions must be given as indicated by the physician on the prescription label.
- Medications may be delivered to the nurse or sent to school with your student. If you send the medication with your student, please email nurse@cannonschool.org so the nurses are aware the medication has been sent to school.
Medication-Related Forms:
● Authorization for (Prescription) Medication Administration Form: Printed Form
Required for all prescription medications administered during the school day, or on class trips.
Print form, complete, have it signed by a physician, and upload to Magnus.
The form must be resubmitted annually.
● Authorization for Non-Prescription Medication Authorization Form: Printed Form
Required if you would like the nurses to administer any over the counter medications to your student during the school day.
Print form, complete, sign, and have it signed by a physician. Upload the completed form to Magnus.
The form must be resubmitted annually.
● Self-Carry Medication Form: Printed Form
If your student requires medication that must be with them during the day, a physician must complete and sign the form.
Download from Magnus, complete, and upload to Magnus.
● Action Plans for health conditions: Printed Forms
These forms are only required if your student has a health condition requiring a plan.
Note: If you answer yes to one of the conditional questions related to a health condition in the electronic Vital Health Record, you will be linked to the appropriate action plan.
All required action plans must printed, completed, and signed by a physician. Upload the completed form to Magnus. The form(s) must be resubmitted annually.

● Athletic Forms: Printed Forms
Students who want to participate in Cannon Athletics at the Middle School or Upper School level must have the following forms filled out by parents, athletes, and doctors. These forms are good for the entire year and all three seasons of athletics.
Upload each form to Magnus, or email to Cannon’s Health Office and Cannon’s Athletic Trainer.
- Participation Physical Evaluation: Print, have physician complete
- Consent to Treat and HIPPA: Print, complete
- Consent to Participate and Release: Print, complete
- Concussion Information Statement: Print, complete
- Sudden Cardiac Form: Print, complete
Health Room Policy
- Students must stay at home if they have a fever of 100.0 and cannot return to school until they are fever free for 24 hours without the use of fever-reducing medication. If a student is sent home from school because they have a fever (>100), they must stay at home the following day.
- If a student vomits and/or has diarrhea, with or without a fever, they must stay at home for 24 hours after their last vomiting or diarrhea episode.
- If a student is prescribed an antibiotic for any bacterial infection, they must be on the antibiotic for 24 hours before they return to school.
- If a student has a skin infection or an eye infection, they must be seen by a provider before they are allowed to return to school. If treatment is necessary, the student may return once treatment has begun.
- Our current Covid-19 policy follows the CDC guidelines in that a student who tests positive for Covid-19 must stay at home for five days beginning with the first day of symptoms. If a student tests positive for Covid-19 and does not have symptoms, they must stay at home for five days beginning with the day of the positive test.
Step 2: Submit the Bus Transportation Agreement (if applicable).
Bus Service: 2023-2024
- Bus Transportation Agreement 2023-2024- submit if your child(ren) will be using Cannon's bus transportation services. Please note that we will not automatically roll-forward your enrollment from the current year to next year - you must opt-in to transportation services.

Step 3: Submit the After School Program Enrollment Form (JrK - Grade 8, if applicable).
- After School Program Enrollment Form 2023-2024 - submit if you wish to enroll your child(ren) in Grades JrK-8 in the After School Program. Please note, the registration fee will be waived for families who complete the enrollment form before August 1.

Step 4: Submit the Grandparent and Special Friend Information Form.
- Grandparent and Special Friend Information - all new families, and any current families with changes to previously submitted information, are asked to submit this form. This will ensure that Grandparents and/or Special Friends are on Cannon's mailing list and will receive Cannon Magazine, invitations to our annual Grandparents' and Special Friends' Day celebration, and other mailings throughout the year.
