Back-to-School Forms for 2019-2020
Follow the steps below to complete the required forms for the upcoming school year. A printable checklist is available on this webpage to help you track your progress. For assistance, please contact the Admission Office at email@example.com.

Bus Transportation Agreement- submit if your child(ren) will be using Cannon's bus transportation services for the first time, or if your child(ren) currently ride the bus but you are changing your bus needs for next year. If you are a returning family and will continue with the same bus service as last year, or if your family does not require any bus transportation service, you may skip this step.
Grandparent and Special Friend Information - all new families, and any current families with changes to previously submitted information, are asked to submit this form. This will ensure that Grandparents and/or Special Friends are on Cannon's mailing list and will receive Cannon Magazine, invitations to our annual Grandparents' and Special Friends' Day celebration, and other mailings throughout the year.
After School Program Enrollment Form - if your child(ren) will be using Cannon's After School Program (JrK - Grade 8), please complete and submit this form. Submit this form by August 1 to waive the $50 registration fee.
