Required Back-to-School Forms 2024-2025
Follow the steps below to complete the required Back-to-School Forms for the upcoming school year, 2024-2025.
New families will receive an email from Cannon School with information about how to set up your My Family Portal account. This is required to follow Step 3 below, submitting health-related information to Magnus.
If you need assistance with your My Family Portal login or the technical instructions below, please email support@cannonschool.org.
- Step 1: Submit the Bus Transportation Agreement (if applicable).
- Step 2: Submit the After School Program Enrollment Form (JrK - Grade 8, if applicable).
