Cannon School continues to partner with Magnus Health to collect and securely store your student’s health and medical information.  Magnus is also where Cannon stores all required athletic and medical forms.  Parents can use the Magnus web portal or the Magnus Mobile App to complete all forms, fill in student health information, and upload forms that need a signature.

Both new and current families can now begin uploading required health forms to Magnus Health. Starting in mid-July, Magnus will send reminder notifications about any pending items on your student's Student Medical Record (SMR). Items requiring your attention will be marked with a "To Do"flag, based on your student’s age and specific requirements.

• We ask that you enter the required health information for each student’s Magnus Health Record (SMR-Student Medical Record) no later than Thursday, July 31, 2025. Timely submission of health information and all required forms is vital to be in compliance with NC laws and to assure everyone is prepared for your child’s first day of school.
• Students who do not have a completed Vital Health Record will not be allowed to attend the overnight class trips. The nurses must have the basic health information on each child to help our teachers and deans safely care for our students on the trips.

New Families: 

• You will receive an email from Cannon School with instructions on how to log into the Veracross “New Family Portal”.
• Once you have set your password and logged in, you can follow the same instructions as current families, below, for subsequent logins.
• Contact support@cannonschool.org with any questions about accessing Veracross.

Current Families (and New Families with Veracross accounts):   

• Use your Veracross username and password to access the My Family Portal.
• Click the maroon Magnus tile to your left to enter the Magnus website.

You may upload forms to Magnus using any of the methods below: 

• Photo:  Take a picture of the form and upload the JPEG directly to Magnus online. 
• Scan:  Scan each document to your computer as a PDF and upload to Magnus.

When you log in to Magnus, on your student’s Student Medical Record (SMR), you will find items that need your attention marked with a “To Do” flag.
 

1. Vital Health Record: Electronic Form:

Information saved from last year will be displayed if your child is a returning student. After reviewing each section and making any necessary changes, please click save to submit for approval. The Vital Health Record must be completed or the student will not be allowed to attend the overnight class trip.
 

2. Athletic Forms: Printed Forms for students in grades 6-12 who will participate in sports.

Students who want to participate in Cannon Athletics at the Middle School or Upper School level must have the following forms filled out by parents, athletes, and doctors. These forms are good for the entire year and all three seasons of athletics.

Upload each form to Magnus. If you have questions, please email Cannon’s Health Office and Cannon’s Athletic Trainer.

• Participation Physical Evaluation: Print, have physician complete 
• Consent to Treat and HIPPA: Print, complete 
• Consent to Participate and Release: Print, complete 
• Concussion Information Statement: Print, complete 
• Sudden Cardiac Form: Print, complete 
   

3. Action Plans for health conditions: Printed Forms - Only for students with a chronic health condition.

• Action Plan forms are only required if your student has a health condition requiring a plan.
• If you answer yes to one of the conditional questions related to a health condition, you will be linked to an action plan.
• Action plans on Magnus are: Diabetes, Asthma, Seizure Disorders, Food Allergies, Bee Stings, Migraines, Heart Conditions, and Sickle Cell.
• All action plans must be downloaded from Magnus, completed and signed  by a physician.
• Upload the completed form to Magnus.
• The form must be resubmitted annually.
   

4. Medication Forms - Printed Forms

• The Prescription Medication Form must be printed, completed, and signed by a physician if your student needs medication during the school day or will need medication on the class trip.  
• The Non-Prescription-Form must be printed, completed, and signed by a physician if you would like your student to take over the counter medication during the school day or on class trips.

• Self-Carry Medication Form must be printed, completed, and signed by a physician if your student requires medication that must be with them during the day.

5. North Carolina Immunizations:

All immunizations must be current before the start of the school year.  In accordance with North Carolina State laws, students cannot be allowed to attend classes if all immunizations are not on file with Cannon School within thirty calendar days after the first day of school. 

• New Students: All new students must submit their immunization records regardless of grade or age.
•  Junior Kindergarten: All students entering Junior Kindergarten must have their 5 year old or Kindergarten immunizations to begin school. Please see the complete list on Magnus.
• Kindergarten: All students entering Kindergarten must have their 5 year old or Kindergarten immunizations to begin school. Please see the complete list on Magnus.  If your child attended Cannon School for Junior Kindergarten, they will not need any additional immunizations. 
• 7th Grade: All students entering the 7th grade or who turn 12 years old before August 15, 2025 must have both the Tdap and Meningococcal conjugate vaccines.
•  12th Grade: All students entering the 12th grade or who turn 17 years old before August 15, 2025 must have the second dose of the Meningococcal conjugate vaccine.
